For your convenience, if you have a Windows®-based PC you can use the FREE PDF-XChange Viewer to fill-out and save these forms. PDF-XChange Viewer is a direct replacement for Adobe Reader®. You do not need to have Adobe Reader® installed to use PDF-XChange Viewer! After completing the forms please print and bring the medical forms with you to our office (some form fields will require you to manually enter your data).You can also save the completed forms to your computer for your records with PDF-XChange Viewer. PLEASE NOTE: YOU CAN NOT save electronically-filled forms with Adobe Reader®. You must use a full version of Acrobat or download and install the free PDF-XChange Viewer if you want to save your electronically-filled form to your computer. You can however, electronically-fill the form and print the completed form with the Adobe Reader®. We recommend PDF-XChange Viewer for its many advanced and useful features, but if you prefer you can still read and print PDFs using Adobe Reader®.
